Accident / incident reports and supporting documentation for dangerous occurrences and incidents where serious personal injury and incapacity (as defined by Accident Compensation & OH&S legislation) of an employee or contractor has occurred as the result of an incident within the workplace or while travelling for the purposes of employment (while on duty or official business), or has occurred to a visitor, student or member of the public as the result of an incident on the University's premises.

Includes radiation exposure incident reports, spillages, contamination and exposure to radiation & chemicals.

Includes accident registers, completed S3 (incident), S4 (incident investigation) & S5 (witness' record of events) forms.

DISPOSITION ACTIONS

- Temporary - Destroy 50 years after incident occurred

- Hold in work unit after administrative use concluded until the records are due for destruction
